This week we met back up with Nathan Windle from Cera to discuss the work we did together last year. This was for a webinar organised by the Population Health Data and Analytics Centre of Excellence.
Cera supports people living in their own homes for longer through more than 50,000 in-person healthcare visits every day. Through a digital-first approach a rich dataset has been generated that is driving improved outcomes amongst their service users. During the webinar we discuss an initiative to predict risk of hospitalisation and hopefully prompt some ideas amongst attendees on how social care data could be used in Population Health.

Our work with Cera covered new initiatives such as predicting hospitalisations discussed in the webinar and also understanding the impact of prior initiatives. An article summarising the benefits of digitising their care plans was published last year.
